The Harbor Health Program of All-Inclusive Care for the Elderly (Harbor Health PACE) is a comprehensive and integrated model of care designed to provide a range of medical, social, and supportive services to older adults with complex medical needs. The PACE model coordinates and delivers all medically needed preventive, primary, acute, and home care services.
Key features of the PACE model include:
• Comprehensive Care: Harbor Health PACE offers a full spectrum of healthcare, social, and supportive services, including medical care, prescription drugs, rehabilitation, personal care, and social services. These services are tailored to the individual needs of each participant.
• Interdisciplinary Team: Harbor Health PACE participants are cared for by an interdisciplinary team of healthcare professionals, including doctors, nurses, social workers, therapists, and other specialists. This team works together to create and adjust care plans based on each participant's evolving needs.
• Day Health Centers: Harbor Health PACE operates day health centers, located at the medical clinic, where participants can receive medical care, social interaction, meals, therapy, and other services. These centers also serve as hubs for participants to engage in activities and interact with their peers.
• Home Care Services: Harbor Health PACE provides home-based services, including home healthcare, personal care, and transportation to and from the day center and for medical appointments.
• Coordination of Services: All aspects of care are coordinated, ensuring that medical treatments, medications, therapies, and other services are well-integrated and appropriate for each participant's needs.
Harbor Health PACE practices the Patient Priorities Care Model. The goal of Patient Priorities Care is to enhance the overall well-being and satisfaction of participants by aligning healthcare services with their individual preferences, goals, and values. This approach recognizes that each patient is unique and seeks to create a collaborative, person-centered care experience that respects their autonomy and dignity.
