My mom was put in a hospital due to swelling in her stomach after she was discharged they placed her in a nursing home for rehabilitation now the social worker that placed her there will not tell me where she is at or want tell me anything about her I am worried about her and just want to know that she is ok and know if she is gonna be allowed to come back home or what? What should I do should what steps do I need to take ?

This usually happens when there is a POA or guardian involved. If Mom has either, only that person will be given info concerning Mom.

Were you involved with her care in the hospital? I would ask the Social Worker why she can't give me the info. If a State guardian has been assigned to her, you will need to contact them to see if you can visit. They now have all the decision making for Mom. If Mom has a POA, they may have asked that you not be told where she is. You need to talk to that person.

are you on her HIPPA Form? Do you or anyone else in your family have POA for health? Is anyone in your family aware, or, are you the last and only family member..
Were you with your mom when she was admitted to the hospital?

Sounds like she may have a legal guardian... I would ask the social worker if she has one and if she does, I'd ask the guardian to be able to visit.

Where does your mother live -- to "come back home"? Does she live on her own or do you live with her or does she live with you or does she live with another family member?

Do you or anyone else have her POA?

Were you visiting her in the hospital, and involved in her discharge planning with the social worker?
