It depends on how near, or what you are calling End of Life.
If they are not actually at End Of Life there can be agitation just as there is at other times.
There are medications that can be prescribed for agitation, anxiety.
If there is pain involved that can cause a person to try to take off a brief, or "mess" with it.
If there is pain or discomfort if the person has been prescribed pain medication "pre medicating" before doing anything that may cause pain can help. So about 20 minutes before the activity that can cause pain give a medication that will lessen the discomfort. (Activity can be anything from changing a brief, repositioning, changing bedding or clothes, )
And if the skin is irritated using a barrier cream can help, do not use a thick coating gently "pat" on a very light amount. the more you smear on the more you have to try to get off later. And if need be leave off a brief to allow the area to dry. A good bed pad and placing a brief over the person can help absorb urine while still allowing the area to get some relief.

Yes it's normal as often a person becomes agitated at end of life and it also can be quite painful to be touched or messed with such as having ones diaper changed.
I would hope that your loved one is now under hospice care as they can give your loved one some medication for their agitation and pain, and also answer any questions you may have.
