is this a transfer that requires the person to remain in a bed?

There are medical transport companies that will do this.
You can check with either the new facility or the one that you are leaving and ask what company they would typically call.
There will be a cost. (Check, since it is a medical transport it may be tax deductible.)
Cost may be determined by distance and how much assistance is needed in the transport. (Wheelchair, then maybe a med van would work, if a gurney is needed then a larger vehicle much like an ambulance would be needed. )

As said maybe the new facility but there will be a cost to the patient. Or you can set up a private transport. Not sure if your local ambulance service would. Ours is volunteer but bill Medicare and Medicare doesn't pay for transporting only. Has to be an emergency.

When I moved my MIL, the new facility was very helpful in getting everything going. They can arrange a transport too, if necessary.
