The law now allows all nursing home patients this option, with the facility PCP serving as a backup.
Seems we may be out of luck in Kansas City cause he is TOO complex and time consuming, even though he is dually insured to take care of the severe neurological issues that have been chronic for decades.
They now include the most severe pain he has ever had to endure (7 to 8 daily on the pain scale despite heavy meds.
When the nurse practitioner repeatedly neglects communicating with family or even is available to his case manager, I am left with no other option except to file a grievance on his behalf.
Nothing “skilled” except providers who play games of avoidance.
yes I have full permission from my husband for me to speak on his behalf.

Nurse Practioners report to an MD. If your husband has pain like this, he needs a pain specialist not a NP. You need to talk to the MD. He must have an office away from the facility.

First question is: Who is this patient's PoA? If he doesn't have one, this may be the reason they are not informing family (due to HIPAA rules). Verbal "permission" from your husband may not be enough... he needs to assign you as his Medical Representative (a HIPAA form) that is filed with his PCP and probably the facility as well. Even if you are his MPoA the PCP's clinic may still want the MR form.

How old is your husband and does he have cognitive impairment?

How long has he been in this facility?

What coverage does he have (Medicare, Advantage plan, supplemental gap, Medicaid, etc)

I'm not in your state, but if the patient can go into a doctor's office with assistance, I would start calling around to different geriatric doctors and pain specialists and asking if they would accept this new patient. My mom sees specialists and some she is taken in to see them and others come to the nursing home.
