I can’t take care of her anymore, it’s too much

We need more info on her condition/diagnosis, how much supervision she needs at home and more information on you and your family dynamics to advise.

Is there a hospital social worker to help you connect to services? Would in-home health aides help you to continue?

Yes, and if she is hospitalized it is a good time to do it. Ask for Social Services NOW. Tell them you cannot care for her at home anymore. Don't listen to the promises of "we can help; we will make this work". They can't and they won't. Don't agree to take her back because "We can't place anyone in covid places; they aren't taking new patients. Just stick to your refusal to take her home and they will get her entered into the system.
I think that you will already have understood that, unless Mom is vaccinated already (and hopefully they are doing this while she is hospitalized--first injection gives from 50 to 90 % immunity by current studies-- she is being subjected to more a likelihood she will get covid. If you CAN hold out and care for her until she is vaccinated you probably should consider trying. But if you cannot, tell the social worker she must now go into care. She will find placement. You will have little choice at present about WHERE that is. But if you are at the end of the rope that isn't now the question.
