Hello, I was just wondering where a safe discharge location is supposed to be? My mom is in a facility and her insurance ran out (long story, there’s no way to get it back), and they gave her thirty day notice paperwork today to discharge. They said they put a random location on the paperwork and for her to talk to people she knew and decide where she wants to be discharged to. I can not take her because I am in another state and she doesn’t have any other family or friends who can take her. In that case, where would they send her? I thought it would be the location on the paperwork but according to them it is random and I called the location listed and apparently they are a private pay facility anyway and my mom cannot afford it. Very confused, have been trying to get in contact with the facility all day but they have not called me back.

How old is your Mom? Over 65?

What are your Mom’s cognitive and mobility abilities? How dependent is she on getting care from others?

Does she have a PoA or a legal guardian? This may be why they aren’t calling you back…

Contact social services for her county and explain this is an urgent housing and care situation for a vulnerable adult, if she isn’t 65 then this is a different problem.

Please provide more info.

If she is not able to live on her own they need to discharge her to a facility that is able to take care of her. I would call APS in her state, tell them the situation, and what facility she is in. They should be able to tell you what the next steps are. It sounds like she needs to apply for Medicaid.

I should have explained better. She was in the facility after a hospital stay, went because of rehab, completed the rehab, and is now able to leave. The issue is she was staying with my family member and they have since passed away, and I live out of country in a university setting so she does not have anywhere to stay in her area.

Helpless, so she is able to live on her own but does not have a home now? She then needs to find an apartment she can rent. What happened to the place she was living in? Was it owned by the family member? Who owns it now? Or was it rented? Can she go back there? If not why? If she is able to live on her own but has no place to go, she likely will be discharged to a shelter. Hopefully the social worker there will help find her housing.

Here's a hint, the random place on the paper, they're doing this to get her off their property. She goes to the other facility (no insurance and/or way to pay), she will be discharged immediately.



You can contact your state ombudsman to see if they can help. If they can, they'll set up a appeal hearing with an officer, Contact your ombudsman, right away, because the hearing is scheduling during the 30 day notice, If she loses, then the facility can proceed with the discharge,

She should be having a meeting with the SW who would be helping her get what she needs so it can be a "smooth and orderly discharge"

You are currently asking a Forum of utter strangers to mom and to yourself where she is going? What would lead you to think we could possibly know? This depends upon:
1. Whether or not mom can function in her own behalf
2. Who the POA is if she cannot
3. Whether or not mom has dementia
4. Whomever placed mom in care originally.

What KIND of insurance was paying for her care and is now refusing to pay for her care. Was this long term care insurance?
Or was this Medicare paying for rehab or SNF?
Or was this Medicaid?

If your mother is being discharged there has to be someone responsible to manage her care if she isn't competent; if she's physically or mentally disabled she can't function to find her own placement.

As to where they will discharge her? That's the only easy answer here:
They will safely discharge her to wherever another facility is found that will accept her.
But acceptance depends, again, upon a whole lot of factors.
1. Whether or not she is competent to manage her own affairs
2. Who the POA or guardian is if she is not competent to manage her own affairs
3. What her assets are
4. Whether she is qualified and has applied for Medicaid or not

Sorry, but you say you are states away from mom. Without knowing who is THERE with and for your mom we can't answer your question. And if we knew, then we could tell you that THAT is the person you need to be speaking with.

Call the Social Workers at mom's current unit and speak with him or her. In lieu of that, call the administrators and speak with them. Again, we are a Forum of strangers who could not conceivable know where mom is going.

You don't mention your Moms age? Does she have Medicare?

Rehab is not permanent. If Mom is 24/7 care and cannot afford it, then a facility needs to be found that excepts Medicaid. You need
to talk to the Social Worker at the location Mom is at and tell them your predicament.

Maybe OP can contact mom's insurance and ask for proof why it was cut (if that has not been done already). Mom completed rehab, then it was probably deemed no "medical necessity" hence insurance cut, that would lead to a denial of Medicaid.

This really is where the SW comes in. Worse case scenario, mom goes to a shelter, then she is on her own. (hopefully it does not come to this). This is why OP and mom need to get on this right away.
