If your bil is on private pay, find where he wants to go, and have the new NH tell you what is required.

If he is on Medicaid, then the rules might be different from state to state. I was told to ask the NH where you are wanting to transfer him into, to contact Medicaid and they will instruct. In my DH aunts case, there is a third party company that has the contract to manage Medicaid. The receiving NH would contact this company and they would work with both NHs to make it happen. Now that would be how to get the Medicaid portion of the payment switched. They called it something like “the money follows the patient”. As far as the patients “SOC” share of cost, if BIL has an ACH transfer from his bank to the existing NH, that would have to be changed. Again, I believe the receiving NH which BIL is moving to should be able to help with this.
I have not actually done this, just had it explained to me today by one of the management companies that works with the NH via their contract with Medicaid.
Hopefully someone who has actual experience will see your post.
