Nocturnal reoccuring explosive watery diarrhea in elderly women can be Microscopic Colitis. The problem is that to get a diagnosis you have to do a colonoscopy with biopsies. Many times this is too hard on bedbound residents.

The treatment after a firm diagnosis is entocort-a steroid. I found this to be a rough drug as a 50 year old female.

OTC management things you can try would be 1/2 immodium pill daily, possibly up to 3/4 or 1 immodium daily (START WITH LOW DOSE) 1/2 immodium pill should be effective daily.

Another thing to try is 2-4 pepto bismol tablets a day.

Use either the immodium OR the pepto for daily maintenance.

The immodium or pepto will not heal the gut. Stay on these for a maintenance dose to make BM's manageable.

The only solution that helped us was to try to contain it better to make clean up somewhat easier. Wearing a pair of shorts style boxer shorts (the kind with snug legs) over the top allowed us to get to the bathroom and into the shower without leaving a trail all the way, tucking pants into the socks adds another layer of protection. I also found that using a wet/dry shop vac or Bissell's little green machine worked very well to clean up any spills.

My mom had this problem on and off for YEARS. It was worse over the past two years and really affecting her quality of life. Her PCP started her on a once daily Imodium and as well as very high fiber gummies and it finally put the explosive and very messy accidents to bed.

Is this every night and watery? It could be Microscopic Colitis. If it is every night and watery I'd see a doctor.

Switch to simple diet: chicken, rice, overcooked veggies. Avoid processed foods. Avoid fried foods. Avoid foods with ingredient lists.

Talk to doctor about discontinueing any meds that cause diarrhea. Many do.

I don't know of any adult diaper that won't leak when there's diarrhea in them, so instead I would try to figure out what is causing the diarrhea and find ways to stop it. Perhaps a visit to the doctor is what's needed here to get answers as to what exactly is going on to cause the diarrhea.
