Perhaps the family is confusing "pureed" foods with "soft" foods that the oranges would fall under.
Like already mentioned you may just have to ask them.

Why don't you ask them? She's your client, and it's your duty to maintain a relationship with her family so that you feel comfortable asking and answering questions of each other. You're all in this together, the purpose being to take good care of the client.

Some people just don't understand the importance but I'll give you another perspective.
My mom was on a strictly pureed diet at the nursing home but since I had been her caregiver for a long time before that I knew she enjoyed and could handle certain other foods just fine. One day I was feeding her some potato chips and I actually had an overly protective dietary aide try to intervene and take them until I set her straight. You need to consider the balance between keeping someone safe and allowing some risk in order to enhance quality of life.

Why not ask the family members themselves who may be unaware of the pureed diet order from the doctor? We are just a bunch of current and former caregivers to elderly loved ones here at Agingcare and we'd have no idea.

Oranges are tough and slippery. I am sorry to mention this, but could they be feeding these on purpose to cause choking? I have heard of this this before with the families of terminaly ill patients who are a choking risk. I have also heard of this being done with other foods, such as ice cream with nuts in it. I think this may happen more than anyone wants to think about.

It could be the family didn't know or did not take the advice seriously. I would ask them.
